How to go about setting up a 504

- If a parent brings in paperwork and requests a 504 plan coordinate with your school councelor and they will set up a meeting with the neccesary people.

- Once you have all the neccesary documents you need to treat the student and have spoken with your school councelor there is nothing else you need to do.

Field Trip Prep

Once you are notified that there will be a field trip coming up do the following things to prepare.

  • Get a list of the student going on the field trip and see if there are any students who take medication.

  • If there are student who take medication going on the field trip contact their teacher and let them know there must be at least one medically trained staff member on the field trip.

  • If the teacher is willing to be medically trained tell them to complete the Vector trainings and either come back to you for check offs or call the Department of Health and Wellness to schedule a check off.

  • After finding out who the medically trained staff member is contact them and go over who takes medications and how to document the medications given.

  • The school day before the field trip create a medication schedule for the teacher.

  • Each students medication should be placed in a bag of its own with a copy of their MAR.

  • Give the medications and MAR's to the teacher the day of the field trip. (Make sure they know to initial the day they gave the medication).

  • Have the teacher drop off the medications and MAR's with you when they return.
